There are two key components to the closure of Dr Hanne's Family Practice: 



What does this mean for General Practice?

General care has been handed over to Botany Junction, Highbrook Medical and Ormiston Medical your patient notes have been transferred over.

They have agreed to accept my patient list and have doctors available who can assist with your ongoing healthcare needs. They are compassionate and competent doctors who are well versed in various aspects of family medicine.

What do you need to do?

General Practice Patients

Your enrollment and patient notes have been transferred to the above-mentioned medical centers. This is to ensure continuity of care and please note they offer high quality healthcare at the same affordable prices as Dr Hanne's Surgery. 

You will need to decide which of these three practices to enrol with, however you can attend services across all three practices, or choose to enrol with another practice.

As required by the Ministry of Health enrolment rules and to ensure ongoing access to low-cost healthcare, you will be required to sign a new enrolment form. These are available online; Botany Junction, Highbrook Medical and Ormiston Medical or in the practices. 

If you choose to enroll at a practice other than those listed above, your new practice will request your patient notes on your behalf which will be sent via secure transfer.

                                  ADHD Patients:



To initiate the transfer your medical notes from my practice to that of your regular GP, you will need to email us at support@drhanne.co.nz with the following information:


1. Your name and DOB

 2. A sentence saying: "I would like my notes transferred to my GP."

3. The name and practice address of the GP to receive your notes.



We will endeavor to do this as quickly as possible.
